A free workshop to promote self-care and positive lifestyle changes to improve the community wellbeing

Aims
Digital self-management programme for older people with the aims of increasing physical,
cognitive and emotional activity by focussing on the three Rs:1. Regain confidence and purpose
2. Recover strength, stamina, skill and suppleness
3. Recondition the body, brain and mind
What is it?
- ٚA small group online learning programme
- 10 ٚ parts, designed to be completed over
10 weeks - Practical actions designed to increase
activation levels and start to build new habits - Personalised support from a health coach
Who is it for?
- Aged 60+
- People with 2 or more long term conditions
- Potential to reduce risk of hospital admission
through increased activity - Funded for 4,000 people across BNSSG
- Those who can be supported to self-manage
Results & outcome measures
- Results to date indicate improvement in all project outcome measures: health confidence, physical activity frequency, quality of life, reduction in sedentary time
- 86% ٚ of completers rate the service as very
good or good

Join us for an afternoon of gentle social gardening activities in our community garden!

FREE accessible drop-in session suitable for older people but open to all patients. All ages/abilities/backgrounds welcome, Accessible garden with seating, indoor space in poor weather Refreshments are provided, accessible toilets
